Coach's Toolbox

• Sign-In Sheet - This chart should be taken to every meeting to keep track of Team Member attendance.  This documentation form is required for all Study Group members to obtain CPDUs.

• Notetaker's Form- This sheet should be completed by one Team Member during each Study Group meeting. Please save each one of these forms and return them all with your final report.

• Attendance Policy

• CFE Absentee Form - If you know you will be absent from a Study Group Meeting ahead of time, please print out a copy, fill it out, and send it to your Coach. CFE will make all final decisions regarding the approval of Team Member absences and reserves the right to terminate grants based on unexcused absences.

• Study Group Attendance Report - This attendance chart must be sent to Nedranae at the CFE office (fax: 312-670-2029) the 15th of every month. For each Study Group meeting held the previous month, simply list the name of each absent Team Member as well as the reason. If you did not hold any meetings, or did not have any absentees, still return the form, but simply note that on the chart.

• Coach Goal Attainment Scale - (GAS): Complete with Team Members and mail a copy by December 1. 2007.  See below for instructions.

• Team Member Data Sheet - For team members to record data results for students.
  Team Member Final GAS Results - Bring this report form and the assessment used to your final Study Group meeting.
• Coach Final Report - Please mail a copy of your Final Report by December 1, 2007. 
• Team Member Final Reports will be mailed and posted the week of November 26, 2007.  They must be returned by December 21, 2007.
